This single volume book entitled "Current Progress in Neurosurgery" comes as an educational initiative of the Asian Australasian Society of Neurological Surgeons (AASNS). Dr Basant K Misra, Dr Edward R Laws and Dr Andrew H Kaye have edited this book published by Tree Life India (ISBN: 978-93-83989-01-0). This book consists of 28 chapters written by 69 authors from across the globe, who are masters in their fields. The editors have intended to provide state-of-the - art information regarding the management of various neurosurgical diseases in a concise way while also ensuring that the topics covered include all relevant details. They have tried to bridge the existing gap between current knowledge and the actual practice of neurosurgery. The chapters have been arranged under four major sections namely Neuro-oncology; Cerebrovascular neurosurgery; Skull base surgery; and, Functional neurosurgery, infection and recent advances in neurosurgery.

The first section on Neuro-oncology comprises 8 chapters.

Chapter 1 discusses the current trends in acoustic schwannoma surgery. The authors present an exhaustive account of acoustic schwannomas starting from their molecular genetics to the current focus on functional preservation after surgery and also includes the ever-growing role of radiosurgery in the management of these tumors. They have written at length about the evaluation and management of syndromic acoustic schwannomas as well. The surgical approaches and techniques have been dealt with extensively. Chapter 2 deals with the complications of endoscopic pituitary surgery and the strategies for their avoidance. The authors present the results of endoscopic sellar/expanded endonasal, trans-sphenoidal approaches at their center and then discuss the various ways of avoiding complications in a systematic manner. This is a very informative chapter and the readers will appreciate it very much. Chapter 3 deals with management of parasagittal meningiomas. The authors discuss the preoperative evaluation laying special emphasis on the non-invasive angiograms and discuss the various nuances involved in their surgical management. They discuss the pros and cons of excision of partially/completely invaded venous sinus and also talk about the role of radiosurgery. In chapter 4, the authors discuss the emerging trends in glioma surgery. They talk at length about various means of intraoperative visualization of the tumor as well as the techniques of preoperative/intraoperative determination of eloquent regions of the brain. They also discuss the role of surgery in facilitating postoperative chemotherapy. In Chapter 5, the authors write about the impact of image-guided surgery in the treatment of malignant gliomas. They emphasize the importance of real time imaging and evaluate the role of an intraoperative MRI (iMRI) by discussing the results of a randomized trial previously carried out by them. As a passing note, they point out the unique advantages of an iMRI over intraoperative fluorescent dyes in the visualization of low-grade gliomas. In Chapter 6, the authors discuss the role of combined microsurgery and radiosurgery in the management of meningiomas. They present their retrospective data analyzing the recurrence rates following institution of radiosurgery immediately following surgery versus the recurrence rates obtained by performing radical excisions only. It was found in their study that there has been a substantial recurrence and increased morbidity after radical resection. They, therefore, argue in favor of an early postoperative radiosurgery as an efficient modality to reduce recurrence. In Chapter 7, the authors present a detailed account of the pathogenesis and pathology of radiation-induced meningiomas, the issues involved in their surgical management and also discuss the role of radiotherapy. This chapter will bring the reader's attention to this hereto relatively lesser known entity that is not often not given the importance that it deserves. In Chapter 8, the last in this section, the authors discuss the microsurgical management of intramedullary tumors. They discuss, in great details, the operative nuances, complication avoidance, and factors influencing surgical outcome in these patients. They also briefly evaluate the role of adjuvant therapy in intramedullary tumors.

Section 2 deals with cerebrovascular surgery and comprises 9 well written and comprehensive chapters. In Chapter 9, the anterior circulation aneurysms are discussed. This chapter presents the operative approaches and nuances in their microsurgical management. This chapter also highlights the philosophy that "clean and fast surgery translates into an improved outcome".

In Chapter 10, a lucid account of the successes and travails in the management of posterior circulation aneurysms is presented. Proper patient selection, decision making in adopting the right approach, and the appropriate operative steps of various types of surgery for clipping of posterior circulation aneurysms are discussed. In Chapter 11, the recent advances in the endovascular management of intracranial aneurysms have been discussed. The chapter takes the reader through various new innovations in endovascular techniques in a systematic way. In Chapter 12, the authors present the indications and surgical steps of different intracranial-extra cranial arterial bypass procedures as well as the 'in situ' intracranial bypasses. The operative nuances are discussed in great details. The authors also present their personal data of various cerebrovascular bypasses.

In Chapter 13, the surgical strategies involved in the excision of brain arteriovenous malformations (AVMs) are discussed. The authors present the surgical principles involved in excision of brain AVMs and then expose the readers to different clinical scenarios encountered and discuss their surgical management. They end by adding a few lines on the future of AVM surgery. In Chapter 14, brain stem cavernous malformations are discussed. The authors analyze their own data and discuss the indications, operative techniques and outcome of surgery for these difficult lesions. The authors incorporate some case examples to depict the commonly encountered clinical scenarios. In Chapter 15, the current management protocols of an intracerebral hematoma (ICH) are discussed. The authors describe the etiopathology, clinical features and management of spontaneous ICH. The current controversies as well as the consensus involving both medical and surgical managements, including the various forms of surgical interventions, have been meticulously presented in the light of current evidence.

In Chapter 16, the authors write about the role of neurosurgeon in the treatment of cerebral ischemia. They describe the pathogenesis, clinical presentation, investigations and management of both acute and chronic cerebral ischemia and discuss the role of a neurovascular surgeon in these scenarios. The writeup is aptly backed up by a thorough review of the pertinent literature. In Chapter 17, the authors describe the indications, preoperative evaluation and surgical steps of carotid endarterectomy and discuss the postoperative complications. The authors also present a critical review of the literature and analyze the various landmark clinical trials published on this topic.

The third section of this book is on Skull Base . This section consists of 7 chapters. The first (Chapter 18) and the last chapters (Chapter 24) in this section deal with anterior skull base lesions, mainly focusing on meningiomas and their surgical management including the surgical approaches and the role of radiosurgery. Both chapters are very well written and are extremely informative. Chapter 19 (Sphenoid wing meningioma) and Chapter 22 (Surgical management of foramen magnum tumors) also provide an elaborate account of the operative approaches, the surgical techniques and focus on the growing trend of conservative resection followed by radiosurgery in these tumors. The standout chapters under this section are "Treatment of cavernous sinus meningioma (Chapter 20), "Petroclival meningioma: Current treatment strategy (Chapter 21)" and "Safe removal of skull base meningiomas: Seven preoperative checkpoints, selection of surgical approach and surgical tactics (Chapter 22). These chapters incorporate the individual experiences of premier neurosurgeons. It describes the various operative approaches and techniques, and provides up-to-date information regarding their optimum management.

In the fourth section of the book, there are four chapters, which deal with functional neurosurgery, management of infections and recent advances in neurosurgery . In Chapters 25 and 26, the authors deal with surgery for epilepsy. In Chapter 25, the authors mainly focus on the operative techniques and complication avoidance in surgical procedures for drug-resistant temporal lobe epilepsy. They also discuss the various factors that determine selection of a given approach. In Chapter 26, the authors cover the advances in epilepsy surgery in children. They review the advances in the preoperative as well as intra-procedural, non-invasive and invasive neuromonitoring. They discuss future horizons like 'connectivity-based approaches' and 'neuromodulation in seizure activity'.

The penultimate chapter (Chapter 27) is an update on neurocysticercosis This chapter is well written and covers all the relevant aspects of this disease. It also addresses the various controversies in their management. The last chapter (Chapter 28) of this book is on "Nano techniques in neurosurgery". Usually chapters like this are very tedious to read as they include advanced physics and other technical aspects that are at times too complicated for a reader without sufficient background to comprehend. But this chapter comes as a pleasant surprise in that it is written keeping in mind the residents who will particularly appreciate its simplicity and coherence. This is easily one of the best-written chapters of this book.

This book is nicely complied and easy to understand. No critical appraisal of a book is, however, complete without highlighting its deficiencies. Although one can understand that it is not possible to include the entire gamut of neurosurgery in a book of this size, one does feel that important topics like "Spine" and Neurotrauma" should have been included in the current edition. One hopes that the editors will contemplate including these important topics in the subsequent editions of this book. One also feels that more illustrations, schematic diagrams and algorithms would have improved the presentation of the chapters of this book. Probably, a uniform format, that incorporates a summary of the salient points in bulleted text enclosed in a textbox and highlighted with a different color, may have made this book a lot more appealing for a quick reference. Moreover, an element of repetition of contents also becomes apparent, especially in the neuro-oncology and skull base sections. This should have been properly edited.

The editors' efforts are really commendable and the book largely fulfills the purpose it was envisaged with, i.e. to serve as a ready reckoner for young neurosurgeons. The book is handy to carry, reasonably concise and includes chapters that will be useful for all academic as well as practicing neurosurgeons. The chapters have been written by eminent neurosurgeons who are masters in their respective fields. The chapters reflect the authors' vast experience. The extensive descriptions of the operative approaches and techniques, and current trends in their management are reflective of the emerging concepts in various neurosurgical sub-specialities. The chapters are followed by an extensive list of references, which a reader may refer to for further information. All the chapters end with key learning points that provide a take-home message for the readers. The operative details have been nicely depicted and are supported by intraoperative photographs, and at places with line diagrams, that will allow this book to be used for a quick reference in the operating room as well. Overall, I think this book will be useful for all neurosurgeons and particularly, for the neurosurgical residents. I look forward to the subsequent editions and supplements of this book in the near future.
