Sir,

Thanks for sending me Prof. V.G. Ramesh's paper "Neurosurgical Teaching in the Present Context."[1] It appears that this has been sent in response to my paper, "Neurosurgical Education: Some thoughts."[2]

Firstly, I would like to thank Prof. Ramesh for his interest in this subject as evidenced by his earlier publications.[3],[4] The purpose of writing my paper was to initiate a debate on the subject. Prof. Ramesh has directly or indirectly echoed my concerns and has provided some suggestions for the future. Most of these were referred to by me. While "virtual reality surgery" or "computer based simulation" have already been initiated at a couple of centers, there is no alternative to "hands – on" experience and appropriate mentorship. Telemedicine is, in my opinion, a poor alternative for training in operative techniques. Prof. Ramesh's suggestion regarding a "central monitoring body" is genuine, but such bodies exist – Medical Council of India, National Board of Examinations – though the role played by them is unfortunately inadequate. May be the Neurological Society of India can play a pro-active role in this regard.

With due humility let me point out that while Prof. Ramesh's primary concern seems to be 'training' and probably rightly so, I expressed my broader concern on 'education'.

The ultimate purpose of education is not diffusion of knowledge or training in skills, but generation of new knowledge.
