Endoscopic third ventriculostomy(ETV) refers to a surgical approach creating an alternate pathway for CSF circulation bypassing the aqueduct of Sylvius through the perforation of the Lilliquist membrane. Although it was developed in the early 1900s, this approach did not find its place in the neurosurgical armamentarium until the development of advanced neuro endoscopic systems. ETV vs. placement of shunt for hydrocephalus is an interesting debatable issue. We should understand that no procedure is superior, both of which complement each other with specific advantages and disadvantages. Also, ETV is not without disadvantages, difficulty controlling the intra-operative bleeding, chances of injury to the surrounding thalamus, midbrain, and basilar artery perforators. In this abstract illustration, we intent on showing the anatomy of the foramen of Monro and the site of the third ventriculostomy.
