Access to Insular region is challenging because of the surrounding eloquent brain and vascular structures serving essential language and motor functions. Along with the surrounding eloquent brain, insula plays a significant role in various cognitive functions. Among various approaches to insula, trans-sylvian approach is the most popular one. Berger-Sinai classification divides the Insula into four quadrants based on the planes of sylvian fissure and foramen of Monroe. The anatomical limits in the insular glioma resection are superior language area and medially the basal ganglia and internal capsule. In this video by Agrawal et al demonstrated the transsylvian approach for the insular glioma. They demonstrated various techniques of differentiating the glioma tissue from normal brain as well as defining the anatomical boundaries. In the above illustration we intend to show the anatomical boundaries of the insula pertaining to the glioma resection.
