Figure 1: Plain CT head shows subarachnoid hemorrhage involving the right Sylvian fissure. The thickness of the bleed is >1 mm suggesting Fisher grade 3 SAH (a). On the reconstructed CT angiogram, there was a sessile dorsal wall ICA aneurysm on the right side which was closely abutting the optic strut without any discernible proximal intracranial ICA (b–d). Removal of the ACP using software tools showed that clinoidal ICA exposure would expose the aneurysm completely and enable proximal control in the clinoidal segment (e)
